The following documentation must be provided for a service contract provider's license:
  • Certificate of Authority
  • Criminal History Questionnaire
  • Financial Security Requirement
    • If a funded reserve account or financial security deposit is being used to meet the financial security requirement, one of the following securities must be submitted: Surety Bond (Form 4002) or certificate of deposit.
    • The funded reserve account cannot be used for any purpose other than to cover the provider's obligation in the state of South Carolina.
  • Itemized list of application attachments, Form 4003
  • License fee of $200
    • The check should be made payable to the South Carolina Department of Insurance. The fee in nonrefundable.
  • Two copies of each service contract form used
  • Service Contract Biographical Affidavit
  • Service Contact Provider Registration Application, Form 4001
    • If the applicant is a corporation, partnership, limited liability corporation (LLC), or sole proprietor, the owner or authorized person of the business must sign the application.
